Frequently Asked Questions - My Health Survey Portal

You can use the My Health Survey Portal to view your upcoming NHANES health exam or telephone interview appointments, and view results from your NHANES health exam. The video below shows you how to use access and navigate the portal.

Accessing My Health Survey Portal

You can contact the NHANES support team at 1-800-344-1386.

Monday through Thursday: 9:00 am–11:00 pm Eastern Time
Friday: 9:00 am–9:00 pm Eastern Time
Saturday: 9:30 am–6:00 pm Eastern Time
Sunday: 1:00 pm–9:30 pm Eastern Time

The law requires that we keep your information private and we take many steps to keep it safe and secure.

We give you a single use, unique code to set up your account.
You choose your own username and password.
Every time you log in we send a new 6-digit code to your email or cell phone number to confirm your identity.
Your portal session will time out after 20 minutes if you are not actively using it. You will get a warning before the session closes and a chance to keep it active.

You can also do things to protect yourself.

Never share your login information with anyone you don’t trust with your personal health information.
Log into your account from home if you have access to private and secure internet service.
Remember to always log out of your account once you are finished.

You may have chosen to receive texts for appointment reminders, notifications, or portal verification codes. Text and data rates may apply. You may opt out at any time. Text messaging originator opt-in data and consent will not be shared with any third parties unless required by law.

Your exam is done by experienced health staff in our Mobile Exam Center!
It may include tests that are not done during routine check ups.
We will check to see if you have been exposed to harmful materials, like lead or mercury.
You get some results right away. Others will be available in a few months.
Our health staff can answer your questions.
You can share the results with your health care provider, if you want.
After the exam, you will get a gift card as a token of appreciation for your time.

Exam and test results will be added to the My Health Survey Portal as soon as they are available to you.

We will send you a message when a test result is ready, and we may recommend that you share the results with your health care provider.
A final report with all of your results will be ready within 12-16 weeks of your exam. We will send you a message by email or text when this is available.
