To create and access your My Health Survey Portal account, follow the steps below.

Create your Account
  • Visit myhealthsurveyportal.cdc.gov on any computer or smartphone
  • Click on “Create an Account”
  • Enter the unique 7-digit account code you were provided by an NHANES Team member. Note: The code is not case sensitive. You will use it only one time to set up your account.
  • Create a username and password.
  • Set up your security questions. If you need to reset your password, these questions will be used to verify your identity.

Wait at least 24 hours
  • If it has been less than 24 hours since you gave us your contact information, don’t try to log into the portal yet. Our system needs time to process your email address or phone number. This is one way we keep your portal account safe and secure. Thank you for your patience as our system gets ready for you!

Go to myhealthsurveyportal.cdc.gov

On the Welcome Page, click “Login”
  • Enter the username and password you created in Step 1 above.
  • We will send you a new 6-digit verification code by email or text, based on the information you gave us to contact you.
    • Every time you log into your My Health Survey Portal account, we will send you a verification code. This helps keep your information secure.
